1. It has come to our attention that the data for Spain, Girona, are for the period 1995-1997 (three years) and not 1994-1997 (four years).
In consequence, the annual population numbers indicated in the population pyramid page 430 are wrong. The correct numbers can be downloaded here. Please note that this does not affect the incidence rates.
2. There were two typing errors at the top of page 88 in the formula giving the crude rate per 100,000:
  1. The sum of the person-years yi and the sum of cases di should be reversed. 
  2. "5 x 1755/12925220" should be replaced by "105 x 1755/12925220".
3. Page 38, table 3.1:

The O&U (Other and Unspecified) category does not include C75. C80 was omitted from this table and should be included in the O&U category.
4. Page 47, table 4.5:
The ICD-O codes for the melanoma category should be 8720-8790, not 8270-8790.
5. There were typographical errors page 72:



  • Line 23: “in the population and / is the” should be replaced by “in the population and Gamma is the”.
  • Line 26: “The estimate of f is given by” should be replaced by “The estimate of Phi is given by”.
6. There was a typing error page 73, left column, line 8:
In the formula following "the overdispersion model...", Gamma should be replaced by Phi.
7. There was a typing error Table 8.1 page 87:
Last column, under row "85+" figure should be 0.50 not 0.30.
8. It has come to our attention (October 2003) that there is a mistake in the population data for Switzerland, Geneva (1993-1997).
